Medication management, also called medication therapy management, is a service provided by healthcare professionals to optimize drug therapy and improve therapeutic outcomes for patients.
The goal of medication management is to prevent medication errors such as underuse, misuse, overuse, or drug interactions. It ensures that patients are taking their medications correctly and that these medications are achieving the intended health result.
This involves a review and analysis of a patient's complete medication regimen and the provision of an individualized care plan. Effective medication management includes:
- Comprehensive Medication Review (CMR): A systematic assessment of all medications (prescription, over-the-counter, alternative, traditional, vitamins, or nutritional supplements) that a patient is taking.
- Medication-Related Action Plan (MAP): A patient-centric document which lists actions for the patient to use in tracking progress for self-management.
- Personal Medication Record (PMR): A comprehensive record of the patient’s medications, which is to be shared among the patient’s health care providers.
- Intervention/Referral: The provider intervenes as necessary to address any medication-related problems. This could include a referral to a physician.
- Documentation and Follow-up: All findings, interventions, recommendations, etc., are documented, and a system for follow-up is established
